FP57 form

Hello,
I have just paid for a Prescription and obtained an FP57 form. The form does not state what to do if you are awaiting a Universal Credit decision. It just says all the legacy benefits and to choose one. I don't want to fill it in wrong as the language on it says you will be prosecuted. Has anyone else had this situation and how do I fill it in?

Once you have your first UC statement you can take the FP57 and your statement (no need to print you can display on phone) into any pharmacy. You can then get a refund.
For filling out the FP57, if it's the old version with numbered boxes on page 2, you tick box 11 (IB JSA).
This follows guidance to tick box K (IB JSA) on a prescription if there is no UC box.
(Expand the Benefits section).
If it's the new version, tick box U Universal Credit.

Yes it's the old version with numbered boxes. I will tick 11 and should I leave a note next to it saying I am awaiting a UC decision?

I get my Prescriptions electronically so no paper form.

Personally when I was in the same position I waited until I had confirmation of my UC award via my first statement.
So when I went into the pharmacy to get my money back I was able to show proof of my UC award.
It's up to you if you want to try before that, the pharmacy staff might not allow the refund though.

No, I will wait for first payment. I just put a note next to box for my own peace of mind. It should be OK. Thank You for your reply.

I forgot to ask what date should I put on bottom of FP57 form? The day I paid for prescription or the day I get refund? Thanks

I'd think it was the date you are claiming refund.

Could you tell me I forgot to mention I haven't yet had 2 week run on of ESA. I havevread you can still get free prescriptions and treatment until the last day of 2 week run on. Could I still ask for a refund and would I still need to put IB JSA? Thank You
