Leaving care home, what info do PIP need?

I am due to leave my care home very soon. To reinstate the daily living component of PIP, what info do they need when i contact them?
I've told my aunt and uncle that i will let them know when i have left, so that they will be there when i ring them.

When I left my old care home and went to live with mum and dad for a while all we had to do was phone PIP and tell them that I was not living in the care home. So all they wanted to know was my new address. They didn’t do a new assessment or anything because we confirmed to them my needs were still the same. So it is just telling them your new address and then they reinstated the daily living component. Then likewise when I moved to my new care home a couple of years later we just phoned them again and told them my new address and that it was a care home and they stopped the daily living component again but didn’t do any reassessment. Hope this helps.

Thank you. It's going to be a while yet. I have an appointment with a OT today to get assessed for mobility aids.

There's a supported living complex next door. I'I'm busy furnishing it.
