Permitted work on ESA, freelance musician

Hello,
I wondered how the supported permitted work form worked if you're freelance? The guy at DWP just said fill the form in saying you'll earn under the monthly threshold.
Bit it says you need a professional support worker to complete the supported permitted work section. I'm a bit confused by it. Do I also need to do this form if I'm a freelance musician?
I receive new style ESA.

Hey there @Flateric you need to fill in the permitted work part, not the supported permitted work. The supported section is only for those who have support workers etc.
