New form for pip review
            hi, I’ve been claiming pip for about 11 years now and have just received my review forms which are completely different from what I used to get. These forms seem to be for first time ever applying for pip which has totally confused me.
One of the questions is asking me about when I last saw a medical professional about my condition. I saw a neurologist about 8 years ago so is that what I out down, I don’t just have one condition either and have only been at my Drs for 3 years and haven’t really seen a dr about my condition as I have just been getting on with things. So I’m worried about this question as well as the whole form treating me like a new claim

Yes the form is shorter buy fill it i giving as much detail as possible.
Add extra sheets of you need to and put your national insurance on each one.
My review is currently underway.
I supplied medical evidence about my diagnosis as at my previous assessments I was questioned about diagnosis.
