Review, couple of questions

I have a number of health issues, there is not enough room in some section to give all details, is it ok to attach a printed document with further details?

Most of my issues are muscular skeletal, although in pain all the time, they get worse with more activities.

Right ankle has bad arthritis, the joint is out of alignment by 6 degrees due to broken tubular that was mis-set after breaking it 40yrs ago.

Arthritis in spine, prolapse L5, right hip replacement 2yrs ago, left hip on the way out. Arthritis in left knee, ankle.

All the above give pain, many of them 24hr, all of them hinder walking, moving, bending and all get very much worse as day goes on with any load on joints, standing, walking, sitting in hard chairs, causing I creased pain levels, attempts to push through with pain killers results in further stiffness and pain that can take days to recover from.

I have also been diagnosed with fibromyalgia.

My question is, how do I best describe this on the form?

Do describe when at worded or best?

    Hi @bubblecatcher ☺️ You can absolutely add additional documents and pages to your form to support with your review. I would suggest when describing this to be as open and honest as possible and provide any supporting documentation. I've added a couple of links below which might be helpful to read through. They give a bit more detail about completing the paperwork

    Thanks for reply.

    Question on walking, walking presents me with numerous issues from different conditions.

    Section asks if can walk certain distance repeatedly.

    My right ankle is 6° out of alignment, has significant arthritis, had one operation already. Left hip on the way, new right hip still gives me pain, left knee bad arthritis, lower back issues from historical prolapsed L5, fatigue from hypothyroidism, fibromyalgia.

    On a good day I can walk 50m or more especially in the morning, less so in the afternoon.

    Bad days getting down the stairs is tough, if I take pain killers and push through pain I can walk a bit further, but suffer that night and following days.

    How do enter this?

    Also I do use cane if needed, but it is not prescribed?

    I did all of mine on extra sheets, I numbered them, added the question and question number, my son wrote a message in each box referring them to the correct page, I made sure to add my name, NI and dob.

    The boxes are so small!
