Requested reasonable adjustments and employer started punitive actions

Hello,

I’m hoping someone can give me a bit of advice. I’m diagnosed ADHD (employer aware before diagnosis) and requested reasonable adjustments about 7 months post diagnosis from my employer as a formal letter attached to an email. At first, my employer said they needed time to consider my requests but within 12hrs started with punitive actions against me. No formal written response given just a spattering of emails my brain could not cope with. I was so upset at the reaction I received. I made this request as I felt I have not other choice to try and get some support. After a period of holiday, I did not return to work. I gave consent in my RA letter for an OH referral but one did not come. My GP diagnosed and signed me off with a disability related illness and noted on the fit note that an OH referral would be beneficial. This still did not work and no referral. I received one welfare voicemail which I responded to via email to the HR person asking again about OH no response.

My GP extended my fit note recently and again explicitly asked them to refer me to OH - again I have not heard anything. I have now been asked to attend a formal absence meeting and I have noticed that my employer has recorded my absence as something else to what was written on my fit note. So, my disability related illness is not mentioned.

Any advice would be greatly received.
