Help with PIP review form structure/format

PIP review form structure help

Hello,

Apologies in advance, I am autistic and panicking having realised I have likely messed up the form and I am running out of time. I am struggling to find the words to explain my situation. And I cannot find anything online, or anywhere locally that can help me.

I have been typing out my answers for the review form based on each activity almost like a story with multiple paragraphs, however, I have now re-read the form and have realised that there are specific questions within each activity.

Despite looking at the form, I have been so stressed that I just didn’t notice the individual questions. And was answering the way I thought I did in my application, and in the easiest possible way for me to write something and get my point across.

I have answered these questions within my text when explaining things, but have not each question specifically.

Will they accept the paragraphs I have written that has the answer dotted within the text, or do I have to rewrite/restructure everything I have written into each specific questions?

Sorry if this doesn’t make sense.

For reference, by activity I mean the daily living/ mobility activities e.g. preparing food and cooking, washing and bathing

By specific questions I mean the numbered questions under each activity page in the form e.g.
26 - Do you need prompting?
27 - Do you need help from the other person?

@potato63 you can ask PIP for an extension in time to complete the Review form. PIP will extend for two weeks. I know a member that got two extensions to complete their Review form. You can give the reasons you have stated and it has stressed you out.

I understand what you have done but I can't say that would be OK for definite as I don't know. Can you add the number of the questions you are answering in your paragraphs? If you have written your paragraphs on separate sheets, can you not fill out the form as normal as well?

I have already received 2 extensions, the first because I didn’t initially know about the review and wasn’t in my flat to get the letter. The second I requested 2 weeks ago because I have been mentally and physically unwell (still am) and basically have a breakdown trying to do anything.

I have typed out all of my answers on a word document as I struggle to hand write. I can’t really number answers to the questions because I haven’t directly answered them and they are all over the place in the paragraphs. I have likely written more than I need to and overexplained but that is the only way I know how to get my point across and avoid missing anything out.

My plan was to type all of my answers, clearly showing which activity it was for, and printing it. I would write a direction in the actual form to the additional pages, clearly stating the section and page numbers etc.

Hi @potato63 - & welcome to the forum.

It's good that you have typed out your replies on separate sheets of paper, which are needed, as you should give as much relevant information as you did with your initial claim form, & there's not enough space in review forms! So, don't worry, as that's absolutely fine, just make sure you say which question you're answering.

If you have done this, then you can simply go back to the form, & with the example you gave, question 26, & tick 'Yes' if you need prompting, then write 'Please see the attached sheets.' If it's just a 'No,' then simply tick that box, & do the same for subsequent questions.

Under question 28, again say, 'Please see attached sheets,' if you've already explained this, if not, the 'why' part may not relevant, but write when the needs mentioned began (an approx. date is fine).

Please make sure you also type your name & National Insurance number at the top of each separate page.

A friend of mine, who is very good with benefits advice, always does her review forms this way.

Sorry if I have misunderstood or I’m just overthinking this.

So, on the form I will refer to attached sheets. But on the attached sheets, can I leave what I have already typed and they can read/see/find anything that answers the specific questions on the form.

Or will I have to change what I have typed on the attached sheets and change it to simply show the specific question and answer?

chiarieds is saying make sure you have stated which question you are answering in your separate sheets and then you quote on the form after ticking yes - please see separate sheet.

Hi again @potato63 - you can still send out what you've typed, which is your explanation about how you're affected; this is still good, & as you say makes sure you don't miss anything out, but, sorry, you do also have to reply to each question directly which also helps you make sure your answers are relevant to each activity.

So, you'll need to also type some more specifically about each question, whilst writing 'Please see the attached sheets' & ticking each relevant box as mentioned above. You had indeed made the right plan, & typing is so much easier isn't it?

@potato63 are you OK now? My last reply was to your reply to me. Then I edited it after reading your reply to chiarieds, where you were not sure and added what chiarieds had said.

Can you amend your typed sheets to include the number of the question you are answering, then you use the form to direct PIP to the answers to the questions. This is what I did and what I take it you are doing with what you said to me in your reply to me.

You can get another extension, one member had multiple extensions which moved her actual review out months!
