How do I attach evidence/ additional sheets to my PIP review form?
Hi,
I am completing my PIP review form and have typed all of my responses. I am going to print this off along with my evidence but l'm a bit confused how or where I should attach it.
Sorry if I don't make sense, I'm stressed and autistic so likely overthinking this.
- I have typed all of my responses and will print them off. When I put the printed paper in the form, should I put the individual sheets on each specific activity page, or can I put them all in together?
- The same with the evidence. Can this all be put in together or do I have to put them on the relevant page where I refer to it?
- Does the evidence need to be annotated in any way?
- Can any of this be printed double sided?
I am intending to hole punch and use treasury tags to keep pages together.

- Put it all in together. Make sure your answers are formatted to include a heading for which individual question you are answering.

- Make sure everything you are sending (evidence & answer sheets) is all labelled individually with your name and NI Number.
- Yes if you want, I personally would then write on each side a page number for example:
2 pages of double sided information 1/4, 2/4, 3/4, 4/4 etc.

The responses can be put together. They do not need to be on each separate page as long as you've given each part a subheading showing which question it is for.
I would recommend writing your NI number and a page number on each sheet so there's no chance of them getting lost. I also stapled mine, though treasury tags should do the same job.
Evidence can also be put all together, separately to the answers. It does not need to be annotated at all.
I would not recommend printing double sided. Firstly it can lead to the back pages getting missed. And secondly it can show through when the pages get scanned making both sides unreadable.
