in Start here and say hello!

Hello I am trying to obtain a PA4form as I have lost the address page of my points which are needed to obtain a companion bus pass.

    Hi @david123_ DWP should be able to provide your companion with a copy of their PA4. They can contact them on 0800 121 4433 or textphone 0800 121 4493 to request a copy.
