I am changing my surname back to my birth name. I recently re-applied for PIP (still to fill in the "How your health condition or disability affects you" form), and was just wondering how I go about telling them of the name change on my application and also the updated name on my bank account details.

Hi just write them a letter informing them about your name change and bank details.

Enclose it with your pip application form and photocopy everything to keep for your own records.
