Hi, I have now been waiting 8 years to move into a 2-bedroom bungalow with large rooms. At present, I live in a 1-bedroom bungalow with small rooms. I need larger rooms because I use a large electric-powered wheelchair and hurt myself & damage my furniture, walls, doors & doorways.

I was advised by social services to ask my GP to fill out a medical form & send it to the Housing Options Team to help me get a more suitable home to suit my needs.

I asked my GP surgery about the GP filling out a medical form & the response was to bring one in to the surgery & they will look at it & fill in & may have a charge to pay, yet I do not know how to get a medical form.

Has anyone else heard of a medical form or been in similar situations? If so, do you know where I can get the medical form from, please?

My 1 bedroom council house has quite small rooms, but the doors are the wider 900mm type, i also have a wet room and wash dry toilet.

Have you ever explored with OT the possibility of such adaptations, if you're having no luck securing a move? I'm sure you're well aware how scare council housing is these days.

Did you not already do this when you first applied 8 years ago?

After such a long time it's probably also worth asking Housing Options for a review of your application so everything is up to date.

They can supply the medical form. It might also be on their website.

This is how I did it 4 years ago.

My surgery also charge for non NHS work.

Hi @disabledkenny have you explored a potential tenancy transfer which might be an option? Your council might have a list you could request. You can find out more here - Tenancy transfers - Shelter England
