Pip review form

Hi I've just received my pip review form I'm on high rate care at the moment and recieved AR2 form nothing has changed an I'm worried about filling the form in incase they take it off me or award me less can anyone advise on do I just say nothing changed on all the forms I'm also in the support group ESA

    Dont just say nothing changed by itself, you can put no change, but then also how you also impacted as well after it. If you unsure, then try someone like CAB to see if they will help you. Likewise provide information they ask for regarding medication, hospital admissions etc.

    So I've got a letter here on what I have had done on my right foot with toe fusion after arthritis caused by my cerebal palsy so if I circle no change but say I have now ongoing issues causing me to have a toe fusion op an these are the medication I'm on after it an that list all my other Dr in now under would that be ok or would they need more

    Did you mention the toe fusion operation and medication in your original pip form or was this after your original claim please?

    My original pip form was in 2016 an I carnt find it however the toe fusion has only just been done in January this year after my CP caused me to get osteoarthritis in my big toe o. Right foot I had the op in January an then may it got infected again after having a infection in January aswell but in may they operated again removed the metal work an 2 weeks ago but the metal work back in after cleaning out another infection hope that helps

    Ice got a discharge letter from surgeon what I've had done shall I attached that to the ar2 form

    @Mary_Scope does my mum write there's no change and I still require continuous help from my mum an partner to do everyday tasks or help me

    @Mary_Scope do I also attached my hospital discharge papers of the toe fusion op ive had aswell so they know I've had that

    Yes it's best to attach as much relevant medical documents as possible as this will act as evidence @nickh

    If the operation has made things worse as you said you now have ongoing issues and if you need more help/support from your mum and partner than before then it's best to say there is change and explain this in detail on the form😊

    @Mary_Scope so if I put my cerebal palsy as caused me to have a toe fusion operation as my toes were curling under my foot an causing pain all the time an my mobility needs have changed an I need even more help now from my partner an mum an il attach the discharge letters I have from January an may the most recent operation

    Do I also write down all my medication I'm on the amitriptyline pre gablin propananol fluxotine lapazole an the blood thinner medication I'm on aswell. Cheers nick

    Yes writing all of that down including the medication and attaching all the medical evidence will show the assessor how your pain, mobility needs and care needs have changed since your original PIP assessment😊

    @Mary_Scope thanks hopefully that will be enough will they refer to my original assessment an medical I had In 2016 I did score zero points on mobility then but high rate care
