I’m so very confused I’ve received the ar1 form for my daughter, I’m her appointee and fill out the forms for her . I’m u sure how to fill the form out . The section where it says are you filling this out for someone else . Do I put me as I’m filling it out. The other questions do I answer them as if I’m answering them such as my daughter needs help with …. Or is it filled out I need help with …. It’s so confusing and there is no guide booklet with it like before .

When you get to the question asking if you're filling the form out for someone else, tick Yes and put your own details down as her appointee. For all the other questions about daily living and mobility, write everything using "she/her" or her name [like "My daughter needs help with..." or "___ needs prompting to..."] so it's very clear to the team reviewing it that you're describing her needs and not your own😊
