Can anyone please tell me from your experience that if the pip assessor ask question can i take someone with me and can they answer the question from reading from the note that has been writen ? Thanks

Yes, you can have someone with you during your PIP assessment. It's a really good idea to have someone with you if you find it difficult to communicate or remember things under pressure, or just want some emotional support.
The person with you can help by reminding you of things, referring to your notes, or supporting you if you get stuck. However, the answers should come from you wherever possible, because the assessor needs to understand how your conditions affect you personally.
If you genuinely struggle to answer, I would explain this to the assessor at the beginning. Don’t worry about having to give perfect answers, take your time and ask them to repeat or rephrase a question if needed.
If you have an appointee, they are able to talk for and answer for you.
