I reckon there be alot of memos sent to different departments since this second review independent had been published.
i phoned dwp up today just to inform them sent more information for my M R. Woman said she would email case manager this. 
Before usual reply is oh put it on system end coversation.
i agree ever assessment should be recorded as then these HP cannot bully people and will have to conduct assessment  if you not read it here is link
