PIP award review

hi has anyone had a form to fill in while on pip called make sure your award is right as still have 12 months left on the date you get your award for it is called a award review and you answer questions on your disability and you answer either your disability is EASIER HARDER OR NO CHANGE never seen this form before but got to fill it in .thanks

I was on low rate both counts for pip. My condition got worst mobility so informed DWP who sent put new form and had a f2f and after a very long delay as DWP behind I was award low rate as before for daily living and high rate mobility. Took three calls to get report worked out award then waited ten weeks for award letter for comfirmation.
