Sorry to hear of your Nan's situation.
Sounds like she needs a manual wheelchair quickly now, so that you can get to the GP etc easier. You can you borrow one from the Red Cross now whilst you get a better solution in place: https://www.redcross.org.uk/get-help/borrow-a-wheelchair
Obviously in the longer term she needs an NHS wheelchair issued to her and Pippa has already linked to that information above. You can ask your Nan's GP to make an urgent referral to the nearest NHS Wheelchair Clinic.
However the NHS will only consider providing a power wheelchair if they assess her as needing it inside and outside of the home. It is worth checking out if the Mobility Trust can assist: https://mobilitytrust.org.uk/
The benefit you need to claim for your Nan is called Attendance Allowance, it's specifically for disabled new claimants over 65 years of age: https://www.gov.uk/attendance-allowance.
Do contact the Adult Disability Team at the Local Authority (council) where your Nan lives and ask them to carry out an urgent assessment of her needs. I would suggest that you also seek advice and support from Age UK https://www.ageuk.org.uk/services/
Best Wishes
