I was awarded pip for 3years end date is next march I received the review form. I suffer from schizophrenia and have done for years as they will no already. My question is what will happen if I tick no to sharing information with professionals I don't trust my physi for reasons I can't go into .I am however sending my prescription list etc which has my anti pyscitis meds on it. I fully expect to go to the assessment so don't overly see the point in allowing them permission to contact anyone and it's not like they will despite my diagnosis etc as been in the system years

The Assessment centre rarely contact Doctors, hospitals ect, its up to you the claiment to provide the evidence and information to support your claim, most recent medical reports not appointments that support your claim that you can or can not preform the PIP descriptiors.0

Will do as I got the later 12 days after the date on letter no doubt will receive ESA forms soon my award ended in march and heard nothing0

You don't have to complete the form alone. Do you have a social worker or not? They can help you fill it out. Remember that the questions have not changed at all. Describe a typical day and mention what you find hard due to the nature of your impairment too. I find it helps to make a list to refer to just in case.
Provide information that is relevant to your claim. List all of your meds, disability aids etc on the form. In terms of supporting or supplementary evidence, things like a photocopy of your care plan are recommended.

Put your national insurance number at the top of the form. Be clear about which question you are referring to in your answer. If you still need help, call or email Citizen's Advice to ask to talk to a benefits advisor. Send one copy off via secure recorded post just in case. I recommend practicing first with a pencil. Make a photocopy of the form in addition for practice.0 -

Nothing much my friend stay whit me and her little dog I got lot pain my arm and my hand I cant do puzzle just watch tv at moments thank you so much0 -
