pip review form

I was awarded pip for 3years end date is next march I received the review form. I suffer from schizophrenia and have done for years as they will no already. My question is what will happen if I tick no to sharing information with professionals I don't trust my physi for reasons I can't go into .I am however sending my prescription list etc which has my anti pyscitis meds on it. I fully expect to go to the assessment so don't overly see the point in allowing them permission to contact anyone  and it's not like they will despite my diagnosis etc as been in the system years

    The Assessment centre rarely contact Doctors, hospitals ect, its up to you the claiment to provide the evidence and information to support your claim, most recent medical reports not appointments that support your claim that you can or can not preform the PIP descriptiors.

    My gp reception won't give me that without FOI request

    If you haven't already asked for the 14 day extension then i'd advise you to do this first thing tomorrow morning because you risk your form not arriving by Monday, if it's not already sent. It's Thursday today and that's not enough of time for DWP to receive the form and for it to be added to the computer.

    Will do as I got the later 12 days after the date on letter no doubt will receive ESA forms soon my award ended in march and heard nothing

    You don't have to complete the form alone. Do you have a social worker or not? They can help you fill it out. Remember that the questions have not changed at all. Describe a typical day and mention what you find hard due to the nature of your impairment too. I find it helps to make a list to refer to just in case. 

    Provide information that is relevant to your claim. List all of your meds, disability aids etc on the form. In terms of supporting or supplementary evidence, things like a photocopy of your care plan are recommended.

    Put your national insurance number at the top of the form. Be clear about which question you are referring to in your answer. If you still need help, call or email Citizen's Advice to ask to talk to a benefits advisor. Send one copy off via secure recorded post just in case. I recommend practicing first with a pencil. Make a photocopy of the form in addition for practice.

    That's not correct, an ESA Support Group award is always ongoing and never ends, unless you've been found fit for work. Your money will continue until a decision's been made on your next reassessment, when ever that will be. ESA is a benefit where you never know when that form will come through your door and it's even more of a surprise because it comes in a white envelope, not brown. This gets me everytime.

    Nothing much my friend stay whit me and her little dog I got lot pain my arm and my hand I cant do puzzle just watch tv at moments thank you so much

    She said I got high dose meds for pain because pandemic not any hospital.i   all happen because anxiety panic depresion I got ulser my stomac she told me drink fluids all this get over she will send hospital to checked

    Bad weather make pain worse morning my body like lock not working at all yea my friend still whit me she look after me today she goes food shopping she drive she told me let we go together but I have lot pain she told me I can bring her mum wheel chair but my house till I go down stairs my body not work thank you so much all scope team all support

    I am not able to write last couple of day because I had bad fall in my house  pain all over my body no body was whit me that time  now little ok my face had bad bruise  my eyes was close swollen thank god much better today

    2week ago I see my gp  now I am only my face big bruise my right side dark i my head bang to floor and my right side face my knee  when I feel dizzy I am on floor  good thing friend who look after me she support me at all.time thank you so much all scope team my friend get me big puzzle but I am fed up because not izzy

    This whether not help at all I cant came out from bed today all over my body pain my friend make me soup.  . She is good support for me I have apt next week hospital she told me she can take me I don't want to go on my own I am panic I hope she came thank you all support

    Hi @meymay93 - I am sorry you are in so much pain. I hope you are able to enjoy the soup. I do hope your friend can take you to your hospital appointment. Could your daughter take time off and go with you if your friend can't?
