I applied in March , there was a discrepancy with verification so once that was sorted I didn't receive the form for weeks. They received my form on 23rd April and I had my assessment today. Call lasted 1hour 30mins. My concern was she had none of my supporting evidence at all, albeit I didn't send it in with the initial form I submitted it afterwards. However I did not feel rushed at all and she allowed me to read out some of paperwork ie DXA scan,  clinic reports to her. Anyway I am glad it's over and will see what happens , first time applying so who knows.

That's probably the reason why. What i would have done was send copies to the health assessment providers too. If DWP received it then it should become part of the evidence used to make a decision, along with the assessment report.

I didn't know I could have sent it to them too. Not sure how the whole process works to be honest. But as you always give fab advice on here I will take that on board. When i sent my form in my local library was still closed so i didnt know where else to photocopy as I have nowhere else local with a copier.  Then they sent me a reminder to send the form back even though there was no date on it to return by so i panicked a bit then obviously I had to send it without the supporting evidence. I didn't have copy of my form at hand today either , I had originally  took pics of pages on my kindle. Bit of a mess I think now
