PIP- assessor advice ignored

Has anyone any experience of conditions being indicated in an assessors PIP report but not being put into the DWP system.
For example an assessor can indicate further help with future claim processes but that doesnt get entered into the system and the person then does not get flagged as needing help with future issues.
This is an impossible situation to remedy sometimes because under certain conditions where a simple M R could resolve the problem that option is denied as is an appeal, leaving only a new claim and the original problem unaddressed.
Any off subject advice or moaning will be reported.

I will explain this seeing as you dont understand.
We have been told that if a claim lapses then because it was awarded by tribunal you dont have any right to either some extra time, a mandatory reconsideration or an appeal as a dwp awardrd claim would allow.
I would like to know if this is definitely true as we were told by the dwp themselves but can fund no info on that scenario.0 -
Actually i described something else that was the result of this situation.
To expl, sometimes with mental illness or other conditions the assessor indicates on their report that a person will need help with future claim processes and in future if theydontrespond to a situation like a claim up for review and there is no response tthis should flag up the assessors indication and cause the dwp to make a bit more effort to see why theres been no response, with phone calls or even a visit.
Hiwever sometimes the assessors notice is not transferred into the dwp system which allows vulnerable people to slip through this safety net and lose their claims for pip through no faultof their own other than having a mental illness that prevents them dealing with things promptly if at all.
In this topic I am looking for info about how this can be fixed, my first reply related to a situation where when this happens and someone loses their claim, in some circumstances such as a tribunal award, they are denied the opportunity to get more time to respond, have an M R or m1ake an appeal, they have no choice but to start again when other people would simply be given more time or other methods of appealing.
Neither situation seems to have much info about it.0 -
Its only intended to deter the same people who generally cause every other thread to get closed, if someone can help they wont be put off because that last bit doesnt apply to them thanks.0
