Hi, my name is maxipants22! How do I get Registered Disabled?

I am very frustrated with the support I am receiving from my Doctor. I hate change but I have a chance to change to move to another medical practice - their application form asks are you 'Registered Disabled'. What does that entail? Who does the registration?

That really surprised me as there is no longer any such thing as bring registered as disabled and you would expect a gp surgery to know that
A disability under the equality act is any physical or mental health condition that significantly impacts your ability to complete day to day activities. The condition should be expected to last 12 months or more -
