LCW Mandatory reconsideration help please

Hi, I have been placed into LCW so I have asked to have an MR.
Today, I need to write out why I disagree with the decision however I’m unsure how to start it, as in do I need to address the letter Dear Sir/Madam or give it a title ‘reasons for mandatory consideration’ if that makes sense?
Such a small detail maybe, but not knowing how to start it is preventing me from doing it and time is running out 😢

Thanks for making this post.
I'm sure that, if you are writing the letter to accompany your MR, there isn't a specific way in which you need to address it, although others may know better than me on that.
I would suggest to address it in whatever way you feel is best, but of course it is probably worth including somewhere near the top that it is to support your MR.1 -
You need to give your reasons why you think you should have been found to have LCWRA. Include a couple of real world incidents of exactly what happened the last time you atempted that activity. Adding detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were. There's no specific way to address it.

If you feel you need to address it to someone then put
TO WHOM IT MAY CONCERN
Put the address of the UC office as shown on the Decision letter.
Quote the reference number they have given you, your name and National Insurance Number.
Also put the date you are writing the letter.
Say that you would like a Mandatory Reconsideration of the Decision not to award you LCWRA on the (date of their letter)
Then go on to write your reasons.
If you use more than one page make sure that your name and National Insurance Number are at the top of each additional page.
Sign your name at the end of it and print your name underneath your signature.
Good luck.

You can also request it on your journal, rather than write the letter and send it by post.

Indeed but the mosy likely outcome will be the decision will remain the same.

Just came back to this post to say that my MR was successful. I have been awarded for 20 months from September 21’ Should be receiving 9 months back pay tomorrow.0
