I have just been sent my pip review form. This is the first review form I have had to fill in. I was wondering if have to have another face to face assessment. quite a few things have changed with my health since my first assessment. Also what if I can't remember when my health conditions started what do I put on the form.

Most people have assessments, which can be by either telephone, face to face or video call. When filling out the form you should treat it as a new claim and give as much information as possible.Include a couple of real world incidents of exactly what happened the last time you attempted each activity that applies to you. Explain in full detail, where you were, what exactly happened, did anyone see it and what the consequences were. You should aim for at least half an A4 side of paper per descriptor that applies.It doesn't matter too much if you can't remember when your conditions started, remember it's not about a diagnosis.0
