ThankyouI am assuming they are taking from 2nd form, he might have to appeal this to tribunal, as he has clear evidence, but then it depending on jobsworth assertion of when request made, he has a valid claim for 2 years, doubt UC would accept 2 yrs fit note without an investigation, especially if they have it on file original form was requested in 2019.But folks need to photocopy these forms and I recommend getting a proof of delivery as well least if it goes missing their end you still have a copy of what was sent0

No, it's not paid at all during the waiting period, which is 3 full assessment periods. It's then paid from the 4th month.
