UC50 form Capability for work

Hi

Can anyone help me understand the points system,is it 15 points in total over all the questions,or do you have to get 15 points for each question? Also I am confused about question 3 reaching ,I don't know whether to tick no or it varies,as I can't get arm above my head. Also eating and drinking question,I can put food in my mouth,but I need help cutting food up,so I don't know what to tick.
