PIP - change of circumstances

Will the change of circumstances form be like the Review form - asking what has changed or unchanged - in terms of each descriptor - since the last renewal - or will it be like a full new claim form? They did not tell me over the phone - in fact, it is my fault because I forgot to ask - given how stressful it is calling the DWP anyway.
I also forgot to ask how long I will be given to send it back - have prepared a lot of the groundwork already.

Usually it will be the longer form. (PIP2) 1 month to return the form but you can ring to request an extra 2 weeks.1
