Short notice pip health assesment with no form

Hi, so ive had pip for a few years now and last week i got a text about a face to face health assesment for next week. I told them i couldnt do that day as my support isnt available so they changed it to the week after. I havent been given a form so im not sure what information they are assessing me on. One of my issues is im not good with processing this kind of thing so i need as much support as i can get. I plan on calling them tomorrow to ask for the form, after that i dont no what to do... i would really appreciate any advice anyone has or if anyone has been through similar, from what i can see its unusual to not get the form. Hi @Lox that does seem very rushed, so I can understand why you're feeling this way. Does the following week give you enough time to put the support you need in place for the appointment? Hopefully you gain a bit more clarity when you call today, and the form is sent to you asap so you have the chance to complete it without feeling so rushed.
I'm hoping there are members who have had a similar situation and can add their advice on what they did/if they were able to get a further extension to allow time to do what's needed so they're fully prepared and supported. Thank u so much for the reply. It is all very rushed and its been really triggering for me. Symptoms worsening cuz of stress n all that. I have been in touch with CAB, they were as helpful as they could be. Apparently, there is no form for me, this is based off pip asking my medical professionals about me a year ago. I don't have a good relationship with my doctor and the didn't contact my Councillor, so I'm very nervous as I don't no whats been said. I also have an appointment with the doctor after my meeting, that's triggering in other ways so I need to change my doctor before…
I have arranged with my Councillor that we will go through the form anyway so I'm as prepared as can be and she will also write me a report to hand over on the day. My partners coming in with me and I've arranged a lift to get there. I'm very nervous and scared but I think I've done all I can for now. I have until the 30th until my meeting so I'm still very open to any advise. Thank u all so much
