I'm fairly sure my GP wasn't contacted for PIP at all.
I failed the first assessment and had to go to tribunal, so I could see all of the paperwork that was used for the decision, and there was nothing from or mentioning the GP in that bundle.

So am I correct in thinking that pip assessors don't need a medical history as it's not needed when comes to daily living ?

Yes, technically medical history isn't needed at all. PIP should be based entirely on functional ability.
Though there are some assessors who will use generic excuses to refuse PIP. One excuse is not being on medication or having a formal diagnosis, meaning it can't be that bad. Or another excuse is that you are taking medication, meaning that is must be managed ok. So you can't really win either way as you don't know what type of assessor you'll get.
From my own experience, and reading many others on here over the years, I don't think there is any way to cover all bases when it comes to PIP (or UC). Just have to do whatever you think is best, send whatever you choose to send, and then deal with the outcome of that in the future when you get the decision, whether that's good or bad.

Thanks. I will write down all medical stuff in the same way as Dr sent it me minus the stuff not relative. As you say it's who you get but then again the ability to write may be put against you.

Oh I don't think that would work as it would show I've copied it.
If I just sent the letter which is great but at bottom it says 'over the page is medical history ' it might still be ok as the important stuff is in the letter?

I don't think they read it that carefully tbh, just skim through for key words, they may not even notice that line. But yes I think it would be better to send the original letter rather than copying it by hand.

Yeh think best to just send the letter and not the medication/ health history list.. I do have brain fog and forgetful.

I don't think it will at all it shows how you struggle and self medicate at times which I've done in my form I put everything you have to

My doctor did my letter put mentally unstable with psychotic outburst that's my medical history that's part of my illness if he just put has moments of anxiety depression they would definitely score down you have to put everything don't be embarrassed put your whole history I used to self medicate to stop the pain of thinking there reasons you did that and it's connected to your diagnosis

My Dr mainly did it to request paper assessment but did put that I have crippling a anxiety and depression that I had to take Diazepam before my last trip to appointment and that she known me a long time. Yes I guess your right but depends who you get and how they see it. I guess I'll get form to fill in so explain more there like how I tried self medication but knew it wasn't thing to do but was so overwhelmed. I already have 10pts for daily living with unfamiliar places.

Apparently yer can get pip if don't have an illness..how does that work with filling these forms in they send. If it's a reassessment after 5 years do they still send a form. I got one last year that asked if I still warrant pip which is why I'm surprised I got this reassessment text.

My GP wrote a letter saying that I have suffered depression and anxiety since 1973, I haven't sent that letter in again as they have it already and it said on the form not to send letters they already have .
I am going to call the dwp Tuesday as i forgot to tell them I need a phone assessment as cannot do face to face , I had a phone assessment last time . It's agony I have huge panic attacks and it makes me cry .

I guess that depends on what you call an illness? For example, I wouldn't consider an amputee to have an illness, but they obviously still have functional difficulty on the majority of days.
They do usually send forms around 8 months before the end date for PIP. If you sent a form last year but never got a response then maybe they are only now getting round to doing the assessment. There are huge backlogs across the country.
