Migrating from IR ESA to UC, do I need to list medical conditions?

Just got the letter to migrate over to UC, I understand under Regulation 19 I do not have to submit a sick note nor undergo a WCA. When starting the claim for UC they ask for a list of medical conditions. Do I have to submit those? I ask because I heard if you submit something (such as sick note) when you don't have to, it can trigger your application into a fresh UC claim rather than the managed migration that I'm having to do.

Yes as part of the UC application you need to list your medical conditions.
