I suggest that you contact the Adult Care Team at the local authority which covers the area in which your father lives. They will arrange for an Occupational Therapist (OT) to visit to assess what home adaptations or equipment are necessary. The OT will be able to advise if your father is eligible for any Minor Adaptations (for equipment and adaptations up to £1000) or for costly adaptations a Disabled Facilities Grant (DFG):

If he is unable to get a DFG and can't afford the work himself you could also look into the possibility of charitable funding:
