How would I get proof of my disabilties?

My Taekwondo Boss wants me to send medical proof of my disabilities to the British Taekwondo Council to support my entry to a Poomsae competition in Barnsley next month.
How do I get these documents? They apparently have to be signed off by a registered Doctor/GP who sees me regularly.
Can anyone help?

Are you able to ask your GP to print a copy of your conditions from your medical record? I've done something similar in the past.

The Office staff are in the process of ringing Woodhouse Medical Centre about that.
I've been trying to ring up myself all week, they just keep telling me to ring up at 8 AM (first thing) for an appointment, due to all the appointments being taken up [Removed by moderator]
