UC request for a fit note that lists for all my conditions

Hi there

Hope everyone is well. Can anyone help me understand thus.

Currently on LCW and UC is requesting a fit note that encompass all my health conditions that prevent me from preparing or working as I reported change of circumstances for health. Has this happened to anyone else?

Do I just list the major conditions or lists every condition? The reason why I asked I should list all my conditions as a few are waiting list for gallbladder removal and waiting for a ultrasound appointment for my foot, my coach told me to report these changes of circumstances. They already know about the majority of my health conditions as they are on their system.

    Hi @Trufflecake sorry no-one was able to answer. Did you have to list every condition?

    You are already on UC LCW. I have never heard of someone needing to send a brand new set of fit notes while you are already on the benefit but asking to be assessed for an 'upgrade' to LCWRA. Tell them this in your journal they have made a mistake.
