Help to explain something on doctors report for claim

My friend is waiting on a decision from the dept for works and pensions on his claim for epliepsy,they asked for copies of doctors reports from 2006  and he recently got a copy himself it states on one page-in your opinion, is the patient's mental state that they require prompting,motivation or encouragement to prevent the risk of self harm or self neglect if so please give details below (only include tasks which cannot be performed without such prompting ,motivation or emcouragement then says no can you explain what he means.

    The doctor has said no?

    If this is correct, it would appear that the doctor doesnt think that your friend is at risk of self harm or self neglect.

    Self harm means deliberate injury to oneself, typically as a manifestation of a psychological or psychiatric disorder.

    Self neglect means 
    people who neglect themselves are unwilling or unable to do needed self-care.  This can include such things as:
    • Not eating enough food to the point of malnourishment.
    • Wearing clothes that are filthy, torn, or not suited for the weather.
    • Living in filthy, unsanitary, or hazardous conditions.
    • Not getting needed medical care.
