HI, I got my long Pip form that needs filling out and it has to be back by 23rd June and I only got it yesterday.
I need medical evidence from my GP, will I be able to get it in time? Soooo stressed!

You can send in medical evidence even after the form has been sent back just put your first and last name as well as your national insurance on each piece of evidence then send it to the address on your letters but do not send originals only send photocopies of the evidence as you'll never get the originals back especially if you have to take it further with a mandatory reconsideration then appeal to a tribunal which can't be done until you've done the mandatory reconsideration I sent my evidence that I couldn't afford when I applied with my mandatory reconsideration

I have a few consultant reports that state my illnesses and pain but don’t really explain how they affect me daily although the conditions I suffer from are pretty self explanatory. Cystic Fibrosis, fibromyalgia, Osteoarthritis in my neck, depression and anxiety but I am hoping my GP will provide my detailed information but that’s sound advice you’ve given me. Thank you so much.

Also have a couple of family members that see you often and know how it affects you write statements but the statement must include their name , address and their relation to you I put my therapist and gp letter and two family statements and my current prescription list my therapist went into more detail than my GP as they just put whats wrong with me and that the severe anxiety makes it severely difficult to leave the house and current medications

Yes my auntie who looks after me massively since my Mum died is writing a letter for me as she attends all hospital appointments with me and also my son who has adhd and Tourette’s, she cares for me when I am bed bound, does school runs along with my sister and partner.
Ive just seen a specialist mental health practitioner and been diagnosed with BPD and complex PTSD so as soon as that report comes in that can go with it too! I’ve got a list of medical conditions as longs as my arm and finding it hard to explain each and everyone on the form. Xx -
She also cooks for me, has me stay round hers with my children when I am really ill, she’s amazing as it my sister and my partner.

I claimed for BPD , PTSD, depression , insomnia and severe anxiety and gotta do the dreaded reconsideration as the assessor pretty much called me a liar and made things up as she went along now gotta wait 6 more weeks to find out even though I claimed in February

If you don't have enough of time to return the form you can ring DWP to ask for a 14 day extension, they will no problems agreeing to the extra time.I would advise that you send all your relevant evidence with the form because once DWP receive it they send it to the assessment providers, a long with all your evidence. If you send evidence at a later date then they will most likely not forward it onto them.You can take it with to the assessment but they don't always agree to take it off you. I've heard a lot of stories where they've refused to accept it during the assessment. They refused during my daughters assessment for PIP. Luckily i'd already sent over 40 pages of medical evidence. This was just extra.Don't leave anything to chance and send it with the form. Don't forget to put your NI number on the top of everything you send. @aaronw3440 you sound very similar to me such horrible conditions to deal with. I’m so sorry that you’re having to prove your illnesses. And poppy thanks for the advice. It’s the same assessment people that put me in the ESA support group without the need for a f2f assessment so I’m hoping on a wing and a prayer they do that again. It also says send it letters about other benefits. Do I send in my yearly esa one that says I am in the support group?

@poppy123456 I understand that they’re both totally different benefits and have totally different descriptors, just wondered why the form says any other letters about benefits would help with the PIP claim? Xx

I have no idea. I just know it didn't say that on the PIP claim form i filled out for my daughter. No harm in sending the ESA award letter, the worst they can do is completely ignore it.
