    You have to give PIP a call to start the claim process on this number 0800 917 2222 and they'll then send you a form  called how your disability affects you and you fill in the booklet and send it off along with any evidence you have which includes , test result,  doctors letter , therapist letters and a couple of family statements who see you often and know how your condition affects you etc and then you'll get a letter about a face to face assessment then the assessors report goes back to dwp to make their decision disability living allowance no longer exists so if you got disability living allowance you aren't garunteed to get pip as the pip system is really messed up
