how do I even start? Do I do a cover letter? I’ve done one but feel like starting again! It’s way too personal..
Is there anyone on here that could view it and see if it’s acceptable before I send it?

Do you mean the MR request letter? If so then you need to look at the PIP descriptors here. https://www.citizensadvice.org.uk/Global/Migrated_Documents/adviceguide/pip-9-table-of-activities-descriptors-and-points.pdfTo see where you think you score those points and why. Adding 2-3 examples of the difficulties you had the last time you did that activity.No one here can help you write the MR request letter because no one knows how your conditions affect you. Only you know that.

No I am aware of that, I’m asking if it’s an acceptable form of letter not the contains in the letter or my condition. Just to see if I’m on the right track.

On the right track for what? You write the MR letter against the PIP descriptors depending on how your conditions affect your ability to carry out the activity.I don't think you're understanding it. Do you have a welfare rights. CAB or law centre near you? If so then i'd advise contacting them for some help.I would appreciate it if members wouldn't tag me please.
