Hi could someone give me an exapme of how long 1 days diary entry should be it says to give 1 weeks to let them see ypyr day to day life how ypur disability affects u if your pain fluctuates how u do activoties safetly but i fimd i am  writing 2 a4 pages for 1 day its far too much so could anyone give me an exapmle just to see how to be doing it right thank you

    Hello and welcome, keep it simple but factual, list the PIP descriptors, washing, dressing, eating, preparing food taking medication, doing any budgeting or paper work and your mobility. Then when you have your list devide it into three time slots mornings, afternoon and evenings. Then write brief notes for each activity, ie, washing took 20mins did not shower, had strip wash. Dressing ok unable to put socks on didn’t bother. Eating, just had toast , cupa soup didn’t feel up to cooking to tired. Tried to use the hover didn’t have the strength in my arms, Medication difficult to open bottles, use blister packs dropped tablets on floor, trouble swollowing pills. Keeping to the facts and not a story is all that is required. Hope you find this helpful.

    OK so the diary idea is out of the window. 
    So in reality all you have to do is pick the descriptors that you should score on and give a couple of examples of what happened when you last attempted them?

    That certainly simplifies the claiming of PIP. 

    For a start I cannot accept that the assessor would accept those examples at face value. I can think of half a dozen examples of what happened when I tried to carry out the majority of the descriptors trying to have a bath in an unadapted bath, trying to cook, failing to eat, failing to take vital medication, falling off an unadapted toilet, failing to dress/undress without help, requiring two hearing aids, trying to calculate household budgets, trying to get to a place that I have never been before on my own, walking 15/20 metres without having support and falling over.

    The assessors that I have come across have basically poo - pooed the whole lot of the above as a figment of my imagination - so much for the examples given and the wasted time in writing them down for them all to be ignored.

    So am i better just writing what descriptor aplly to me and give examples and leave the diary oit of it all together like yesterday  i was makimg dinmer went into livingroom to get something and blacked out i had the  deep fat frywr on and was so lucky my eldest son was there to switch it of and try and rouse me so just put that in cooking and how i couldnt preform the task safetly and need aupervision this is the 5th time in 2 weeks this has happened to me not cookimg but doing diff activities i am hoping my consultant will rewuest maybe head scan as i am afriad of something serious happening when i am alone

    Thank you as i habe fallen down stairs and hurt myself it really is random the only warning i get it a feeling of extreme fatigue a sharp pain in head about 2 secs then gone i wilk ise all the onfor u gave me thank you so much it has beem so helpful

    Yes  he has had me on so many diff meds from feb now im on co codamol 30 /500 2 evwry 4 hours tramadol 100 mg 3 times a day gabapentin 300 mg 3 times a day amd buspirone 5mg 3 times a day this last one just for a week he has refered me as urgwnt to rhymotology urgently  but 8 month wait list he only refered me October  so i have booked a private one for feb 2nd as i cant wait that long he thinks i habe fibro amd a form of auto immune disease that is atracking my bones that is gene related so i really habemt got  a clue i have told him about the episodes of passing out and falls and tje chronic pain i am in even with all the drugs he has goven me nothing is working he is worried himself as he said it is so rapid and i am deteriorating so quickly nearly every day i am in agong amd can hardly move my upoer body siezes to point i am frozen and migrime is like my brian is trying to oush through my skull and afdwcts my face u cant touch it i am so sensitive all over my mum keeos asking mw to go to A and E bit what is point  as they will say u need to get refered which i am and if my meds r not working they wont be able to do anything plus  all A and E are over run so its not fair for me to take uo their time welk thats what i think abyway it is just i am so down as i cant get any beeak from oain at all and i cant sleeo i get 2 to 3 hours a night and have niggtmares i thought i was drowning and my son shook  me awake as i was holding my breath in my sleep think my head is going lol

    Oh I'm so sorry to hear this @mumof22018, that sounds really difficult. I'm glad you have your family to support you and that you have an appointment coming up sooner rather than later. I really hope it gives you some answers, and in the meantime, we're here to listen.

    If you do have any urgent concerns about your health at any time, there's also NHS 111 which you can call to speak to a trained advisor.

    Thank you all so much its nice to bw aboe to tslk to peoplw who understand that isnt your doctor going to attwmot to put uo xmas tree today norm have it up and all jist havent had the energy or go to do it moght cheer me up and thanks everyone will keeo in touch let u know what consultant tells me x

    Ok so i did the application and typed answers ar top of page put name dob and ni no

    I answered every question and gave 2 examples for each where i was who was with what was doing what happened and how it affected me i attached a list of my meds and side affects
    Thrn sent it as had to be in 14th jan 
    I have my rhymotology app this sat do u think he would habe a report right away?  I want to send a copy of what he thinks
    I have my OH work app 18th jan again i will get copy of report and if any reccomnedations can i send that ? 
    I have requested a copy of my medical history for the last year and sicklines for last 7 months ive been of work so was going to copy and forward these 
    Doc has tried new meds said if they dont wrk will refer me to nuroligist ao again i can get a copy of the letter to forward aswell obviously i wont have all this in time for my f2f unless im lucky if i do is any point taking the copies to that and giving assesor them ? Or should i just forward to dwp and capita registered post 
    Or should i wait to after assesmnet and if turned down forward it with MR 

    Know thats alot just am on fb they have pip group there and there is alot of different conflicting messages one saying i shouldnt have applied as im nor formally diagnosed with fibromyalgia yet and dont really ha e evidence from  specialists

    Forgot to say i had 3 A4 typed pages of answers to the questions i answered was quite alot maybe too much? But wanted to give 2 examples and explain

    Hi,
    Firstly, you don't need a diagnosis, to claim PIP because it's not about a diagnosis, it's how your conditions affect your ability to carry out daily activities based on the PIP descriptors.

    You won't be able to have the Rheumatologist's report on the day of the appointment. It takes a few weeks at least for the report to be typed and sent to your GP. You can certainly ask to received a copy and one will be sent to you at the same time as your GP.

    Sicklines? If you mean sick notes then they're not classed as evidence for a PIP application because PIP isn't about not be able to work. People claim PIP and work.

    Future appointment letters are also not classed as evidence because all they say is you have an appointment in the future, they don't give any information.

    You can send in extra evidence but to be safe i'd send it to DWP and the health assessment providers. I wouldn't take any chances of being able to hand it in on the day of the assessment because some HCPs will refuse to accept it on the day.

    I'm not sure why you would hold evidence back for a MR, if you have it, send it.

    Evidence to support how your conditions affect you, this can be letters from those that know you well, a diary written by yourself. Did you send anything with the form, as you said you've already sent it.

    Yes i sent 3 A4 typed pages to cover all the questions giving 2 exapmles foe each who was there what happened etc as mum didnt get her letter in time written how she cares for me and cut her hours of work to 16 to do it as i need so much help now i also made sure i talked about my aids walkim stick stool for kitxhen bath rails although ver rare used now as i cant get one until someone able to helo me and potable toilet for livingroom as i cant make it to toilet in time also had to  recentley purchace walking frame as i lean to much in stick so still am falling unfortunately it helps balance me
