@Mfarrell Yes unfortunately you do have to put everything down on the form as that is what will be used for your review. If you have a copy of your first form it's easier just to copy what has not changed and add on any new information.

Hello, thank you for your reply yes I have copy of my last form I will copy what I can,
Many thanks -
Hi.You also need to make sure you send evidence with the form to support your claim because they very rarely contact anyone for this. Be aware also that there's no extension of time given to return the form and it must be returned no later than the date stated in the letter. Good luck.I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying. If i see a question that i know the answer to i will try my best to help.

Hello poppy thank you I have contacted my GP for an up To date letter ,I have to write down what I want the letter to say and the GP'S sectary will type me one,it costs £11 ,I also have a copy of the letter I got in 2017,I feel a bit of a fruad saying what I want the letter to say,but this is how my practice works, many thanks for this forum it feels good to know I'm not alone,I'm new so hope to be able to help others if I can.As my disabilities change due to deterioration I dont have much new evidence to send, once again thank you

A patient lead letter from your GP where you tell them what to write isn't classed as evidence because you're telling them what to write. Do you have any other evidence you can send? It's free to request digital copies of your medical records from your GP.
I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying. If i see a question that i know the answer to i will try my best to help.
