what does this mean even tho the points counted up show 8 points for mobility 8 for standard care

That's the box where the HCP has to write a justification for the descriptor choice. As there's a recommendation for 8 points then the box should not be ticked. Underneath that there should be reasons why that descriptor has been recommended.

...but the poster has it on their report that the assessor found no functional restrictions affecting the mobility activities.
Maybe a case of the left hand no knowing what the right hand is doing? Mind you this is typical of such a report - complete nonsense.
