What medical information should we request

My friend is applying for PIP, first go around.  We decided to gather together all the Medical information needed before applying for the PIP form and wording our answers before filling in the form. I helped another friend some years back but forget now what is needed.

She applies to her GP for her medical records. What should she be asking from them? Do they have copies from specialists ? How many years to go back, I’m guessing 3 would be enough.

I know people say everything but there is such a thing as information overload.  A list of necessary information to ask for would be appreciated.

    Hi,

    Most medical records are not of much use because they rarely state how your conditions affect you. A recent prescription printout list could be useful, possibly a diagnosis letter, although it's not about a diagnosis, it's how your conditions affect you.

    They could request the medical records from their GP which will be free for the digital copies. Once they receive them go through them to see if there's anything useful.

    Less is often more for evidence. Sending huge amounts of evidence is not helpful because they most likely won't read it all.

    The best thing that you can do is fill out the form with as much information as possible and then add a couple of real life examples of what happened the last time they attempted that activity for each descriptor that applies to them.

    Hi and welcome to the community, as Poppy has said less is more.

    I only sent with mine a copy of my prescription, a care needs assessment from Occupational Health a letter from my consultant giving diagnosis and treatment.

    All the rest was just me completing the form

    hi kah22, i have recently started a new claim for pip too. i agree that less is indeed more. i sent off a prescription list, consultant letter and a letter of support written by my gp explaining how my illnesses affect my daily living and mobility etc . good luck .

    thanks mike, i filled in the form myself the best i could . i didn't put examples of what happens though , i just tried to explain everything as clearly as possible . i'm a bit worried now that i have messed up my form and my chances of my claim being successful , i will just have to wait and see .
