Are you wanting to change your plan if so contact the team who put it in place was it social services
What sort of plan is it and who provides your care would be good place to start -

It's normaly social services that deal with a care plan if a carer believes that a care plan isn't working properly or if more care is needed or some aspects of a care plan aren't required it is reported to the manager who then reports it to social services who then review the care plan
I am aware that social services have been reducing care hours due to cuts
