Lcwra form. In terms of when illnesses began, should I put when I was first diagnosed?

Where the form says to list illnesses and dates when they began, would I put 1991 when I was first diagnosed with depression (been on anti depressants since then) or the date that it has incapacitated me from this time please?

    I would assume the date you were diagnosed would be sufficient.

    It's not really about a diagnosis so dates aren't really vital and they don't check anyway. What's more important is the information you put down.
    I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying.
