Please put your postcode into the link provided in an earlier comment and this will tell you what advice agency is local to you. We can't tell you exactly what you need to write for the MR. We can only advise you on things that you should include. You need expert advice to check that you qualify for certain.

It's against the rules to put personal information here on the community. Please use the link yourself and choose the topic "welfare benefits"https://advicelocal.uk/

Yes I did call local citizens advice but they told me they need to see my letter before can make any help or comments. But I am waiting for my decision letter to come.0

Ok thanks0

Is it better to read medical assessment report because she put lot of thing on her own word or better to wait for original decision. Can you please tell me. Or just follow the original decision and write down.0

Hi @Bhuyan - when you have an assessment you say how your disabilities affect you, & the assessor then writes down what they think in their own words. The assessment you had was not a medical, just trying to find out about how your disabilities affect some activities of your daily living and moving around. You will need to wait for your decision letter to see what that says. Please let us know when you get this; you can not do anything until then.0

The report from my medical assessment about my restriction is untrue. I did not tell her I can work 75meters. She asked me how far you can walk as I told her I can walk 30 meters to 40 meters but not more than 50 meters and how far from your house to your car. I told her around 150 meters to 200 meters. And she asked me can you walk without stop, I told her no. I need to stop and walk and stop and walk. But where she gets 75meter I don’t know and, assessor put in her report I need to stop 45 seconds it is untrue again. I told her I don’t know how long I stand but may be 10 seconds to 15 second.

You didn't have a medical, you had an assessment of your functional needs. As advised, the assessment process is not there to write what you say word for word. It is for them to gather more evidenced to support your claim. They then write the report, which is their opinion of everything you sent and what you said during the assessment. As advised many times here, please use the link posted to see what agency is local to you for further expert advice.0

This will have caused problems because it's a contradiction.Bhuyan said:

I did call citizens advice they told me they can’t help but they told you can write in your on word and explain them why you don’t agree0

First thing you need to do is put the report to one sideand forget about it because this isn't going to get you a PIP award. Then concentrate on the anecdotal evidence (real world examples) as previously advised in this thread.

I write some examples but again my writing skills is not good0

Can you use a computer/laptop to type it and then print it out? Make sure you add your name and NI number to everything you send. Or you can use this form by filling it in online and then print it out.

Can I point out how many mistake assessor done on her assessment report.0
-
Best not to comment on 'mistakes' in assessor's report. Put this report away, & concentrate on the activities you feel you should get points with, & give a couple of detailed, recent examples of the difficulty you have either doing, or trying to do them.

That is not going to get you a PIP award.

Can I comment which point assessor don’t agree with me. Can I give examples and probe from my medical report0
-
You need to put the report away and forget about it completely. Then give the real world examples for each descriptor that applies to you.
