Received PIP renewal form, but my award said it didn't end till november 2022?

hi i have just receieved a renewal form for my PIP. it said it was awarded to me untill november 2022. is there any reason why i have to renew it so soon? my condition hasn't changed, it has got worse and at the minute i dont have any support i was with EIP for over 4 years but my care ended, as it is only a 3 year support they offer, i  have evidence of my condition and supporting documents etc, i just dont know how i can write it out as i have a hard time understanding alot of the questions and i use to have a support worker but dont have one anymore.

im pretty stuck with how to sort this out, can i just send all tthe evidence that i have and will that be okay or does it need filling out

    Was your November 2022 date an extended date given out last year in response to COVID? If so these extension were to give DWP time to reassess claimants before the claims ran out.
