I have just applied for pip for the 2nd time in 4yrs I decided to try again as my condition has got worse . I worry about being completely honest because I feel they think I'm over reactioning about my conditions.

OP, don't worry about what you think the assessor and DWP may think about what you write. Your task is to describe as fully and truthfully as you can how your difficulties affect your ability to carry out the PIP activities to the required standard. See tips here

But, as the others have explained, exaggerating the effects of your condition isn't the right approach. Did you receive that advice from someone?
I hope the above comments have helped you to understand how to go about applying for PIP, and have put your mind at ease a little in terms of what the assessor or decision maker might think @mbolt. Do you feel as though you'll be able to gather together some evidence in line with the descriptors?
