Who signs the firm

My son has recently applied for UC and his work coach has told him to apply for LCWRA due to his disabilities, the firm was sent out to my mother (his Nan as his mother has passed) I will help fill the form out but didn’t know if my mother or son signed it

If the form arrived addressed to your mother then that suggests she’s his appointee so she will need to sign the form.1

Sorry another quick question, do I fill it out as if I’m my son or on his behalf, for example do I put I can’t cross the road safely, or he can’t cross the road safely

As my daughters appointee i will always fill in those forms in as if i'm her. So for example, if he's unable to cross the road safely you write, "i'm unable to cross the road safely because"... you need to make sure you give a couple of real world examples for each activity that applies to him. Include detailed information such as where he was, did anyone see it, what exactly happened and what the consequences were.I always use extra sheets of paper because there's never enough of room on the forms to give all the information. Make sure you include his name and NI number on everything you send.1
