Hi, My wife is disabled and in the process of re-assessment form. three times I have had to to tribunal so trying to get the form completed as much as possible.

Sorry should have said. It's for PIP. It's really difficult every time she has an assessment so making sure I fill in every box I can. Last time filled out no change and got 2 points I think.0

I can see why only 2 points were scored if you put no change and you have to keep taking it to Tribunal.Putting no change would mean there’s been no change since DWP made their last decision. There’s no guarantee they would check that Tribunal decision.
All claims need to be treated as a new claim. You need to put as much relevant information as possible, without telling her life story.
Include a couple of real world examples of exactly what happened the last time she attempted each descriptor that applies to her. Adding detailed information such as where she was, what exactly happened, did anyone see it and what the consequences were.Use extra sheets of paper because speaking from plenty of experience there’s never enough of room on the form to include all the details. Make sure you put her name and NI number on everything you send.0
