How do I claim Higher Rate Care DLA for a child who is already receiving Middle Rate Care?

My child has been getting Middle Rate Care DLA for a few years due to ASD. They are now being investigated for Crohns Disease which has meant a lot of hospital appointments, invasive investigations plus they are in constant pain and anaemic/weight loss. This has meant a much larger care load for me plus dealing with hightened anxiety which has made their ASD much worse and they are spending a lot more time calming them and dealing with more physical issues (diet change, medication etc) plus more mental issues.
I want to apply for the Higher Rate Care but I am not sure how to go about this. Can anyone advise?

You would need to ring to report changes and you will be sent a form to fill in and return.I would appreciate it if members wouldn't tag me please.
