Hi, would I be able to claim (f.) for this activity because of extreme fatigue. If I can manage in a morning but due to the effects of PBC Liver disease and increasing fatigue I could not manage to do it by mid afternoon.

No, following and planning a journey is for those with mental health, cognitive or sensory issues.Fatigue would come under moving around part but it would depend on how far you can walk, as that applies to physical conditions.1

Okay, thanks poppy123456. My review form has just arrived this morning. Is it okay to add extra anecdotal evidence typed up on extra paper, adding name, address and NI number on each sheet or would I have to send it handwritten.0

Good luck. I submitted the whole form for my daughter typed, Under each descriptor I just wrote see attached. She was awarded straight after assessment. I did give them a reason in any other information why I typed it. She has ocd contamination issues so couldn't touch form.
