Hey,
My review form finally came this morning! It’s been over a week since they sent the text but it’s here now so I can finally get it done and sent off.
section 2 asks for a health professional who knows about your condition best details. Can I put my private therapists? I have recently changed drs surgery and to be honest, I don’t really see a dr much not about my disability. It’s something that’s managed by my therapists so they would know more.

If hey're the health professional that knows he most about you, that'd make sense.
You can always add your GPs details on a separate sheet of paper and add your NI number and signature to the top edge of the page, just to be on the safe side. 0 -
